Central Only

Necklift

Sometimes people are born with an 'obtuse" neck angle, and even from a young age have never had an ideal neck contour. This can be due to the position of bone, cartilage, fat, and muscles under the chin. Portland facial plastic surgeon Deniz Gerecci, MD can perform a deep neck submentoplasty to remove deep fat, re-secure muscle, and re-drape skin into a more favorable position. This leads to an improved neck and jawline contour

AM I A GOOD CANDIDATE FOR A CENTRAL ONLY NECKLIFT?

A central only necklift (also called deep submentoplasty) is a method that addresses fullness in the central part of the neck (directly under the chin.) It does not address jowls or pull loose skin. It is ideal for patients who have a hereditary fullness in the central neck and have an obtuse cervicomental angle. It tends to be more common in patients in their 20s and 30s who have minimal skin laxity or looseness. The procedure includes neck liposuction, removes deep submental fat, and suture tightens the platysma muscle in the midline. A consultation can be a great opportunity to learn whether this limited type of necklift is the ideal treatment to help you fulfill your cosmetic goals.

HOW IS A CENTRAL ONLY NECKLIFT PERFORMED?

This procedure is best performed with you asleep (general anesthesia) at the surgery center. Dr. Gerecci obtains access to the fat in the neck using an approximately 2-3 cm incision hidden under the chin crease. The deepest layer of fat beneath the platysma muscle is removed. The platysma muscle is re-sutured in a more favorable position. Most of the time liposuction of the fat underneath the skin surface is also required. The entire procedure is done in a conservative, gentle manner to leave a smooth contoured neck.

WHAT TO EXPECT AFTER A CENTRAL ONLY NECKLIFT

This is an outpatient procedure. Once the procedure is complete, patients can go home with a compressive head wrap that is worn for 1 week. The sutures will remain in place for one week. Sometimes a drain is placed. Patients can expect swelling and bruising around the surgical site, which should both resolve after the first week. Makeup and skin care regimens can be restarted on day 10 after the procedure. Patients usually have minimal downtime before returning to work.

ARE THERE NONSURGICAL ALTERNATIVES TO A CENTRAL ONLY NECKLIFT?

A central only necklift is recommended when a full necklift (which includes lifting and excising excess skin from the sides of the neck) is not required, but neck liposuction is not enough. It is a simple procedure that gives permanent results and minimal downtime. There are some injectable medications and non-surgical devices that can be used to minimize or reduce fat in the neck, but none will achieve the dramatic improvement in the neck and jawline like a surgical procedure can.
